When calling for an appointment please verify that we participate with your insurance plan.

If you would like to schedule an appointment for a newborn, he or she must be added to your policy in order to be seen. This does not happen automatically. You must call your insurance company and add your baby to your policy.

You will be responsible for any co-pays, deductibles, remaining and non-covered services at the time of the service. Always bring your valid insurance card for an appointment.

If you have any change in insurance coverage please notify us as soon as possible. We do not file secondary insurance. Please inform us if you need documentation to submit to your secondary carrier for reimbursement.
