How to Schedule.

To schedule an appointment for you child, please call: (480) 677-4545 between 8:00 a.m. and 5:00 p.m. Monday through Friday. On Saturdays, phones open at 8am until the schedule is full for the day.

What to Bring.

  • Insurance card
  • List of all medications being taken
  • Recent immunization record (new patients only)
